How can they submit claims with no procedure code but just put Misc instead of code. Of course it will be denied. Ins co should question no code claims and put denied until they know what patient is being charged.

Smoothwaters : You need to contact your insurance company. Mine is always happy to tell me of coding errors by the drs. office and they even sometimes give me the correct code to give the office. It kinda sounds to me like the office does not have a qualified person doing their billing.
